Claim Box

The Claim Box (figure 5) contains several important items that are necessary to produce claims correctly.

Included here are

•      Where the claims should be sent

•      Whether or not the patient has signed an Authorization to Release Information to the insurance carrier

•      The Release Date of the patient’s signed authorization

•      The Claims Cycle

•      The Form Type to be used for this patient’s insurance claims

•      Whether or not to include the ChiroPadEMR SOAP notes for the dates that match the claims. This should be done ONLY if the claims are being printed on paper.

•      If there is another insurance carrier, if the claims should automatically resubmit to the next carrier

•      The automation of Transfers and Write-offs

•      A unique Claim Number that is different than the patient’s Policy Number
