Claim number

Some insurance programs (especially automobile and worker’s compensation), instead of or in addition to a policy number, assign a Claim Number. The Claim Number is used for specific charges and dates of service. The Claim Number identifies the patient to the insurance company and shows that the services and products are related to a specific incident. The Claim Number may be provided by the insurance carrier during any verbal authorization process, in a letter from the insurance carrier, or it may be the same as either the Policy Number or Identification Number found on the patient’s insurance card.
