Alternate Treatment Facility
The information on this tab (Figure 38) is used by ChiroOffice for populating information on the CMS-1500 claim form, box 32. If this provider does give service to patients on a regular basis at a facility other than the office, place a check in the box and then enter the information in the boxes on this screen.

In order to activate the Alternate Treatment Facility window, place a check in the box labeled “This provider treats patients at an alternate treatment facility.”
Be sure to enter the name and address of the alternate facility. It is critically important to include the Facility’s NPI number, not the provider’s or clinic’s, in this space.
If the practice has been issued a taxonomy number, be sure to enter it with the appropriate code.
