This box contains 4 critical items/numbers that identify the patient’s claims to the insurance carrier. There MUST be an entry in each of these boxes. If any of these 4 boxes is left blank, the patient will be listed in the Pre-Billing Tab of the Insurance Manager which will hold and NOT produce claims until all of these items has been entered.
