Insured’s policy group or FECA (Box 11)

This is the Insured’s policy group or FECA number for this policy. The group number is usually found on the patient’s Insurance ID card. An entry here is REQUIRED. If there is no group or FECA number, then type in either [1] capitalized N/A which will appear in the box on the CMS-1500 form or [2] lower case n/a which will leave the box blank. If this box is left blank, the patient will be listed in the Pre-Billing Tab of the Insurance Manager which will hold and NOT produce claims until this item has been entered.
