Use patient information for remainder

If the relationship to the subscriber policyholder is NOT Self, and the patient’s Address and Phone are the same as the policyholder’s, then place check in this box so staff does NOT need to spend time typing it. The computer will use the patient’s address and phone number for the remaining fields if this checkbox is active. If this box is not checked, you must enter the policyholder’s address information.
