T.I.N.

This is the provider’s State tax identification number. This may be identical to the E.I.N. or it may be a different number issued by the state the practice is located in. Regardless of whether or not it is identical to the EIN, it must be entered in this box.  If the practice does NOT have an T.I.N., contact the State Tax Authority to obtain one.
