The Time of the SOAP Note should reflect the time that the patient was in the office. If the note is being created at a different time than the visit, it can be changed by clicking onto the hour, minute, and/or AM/PM entry and just type in the correct time. There is also the option to use the up/down arrows on the right side of the Time box.
