Relationship to Insured

At the top left is the drop down box to show the Relationship to the Insured person. There are 4 mutually exclusive choices: Self, Spouse, Child, or Other. The default is Self. If the actual relationship is Self then there is no entry to make in this box. If the relationship is Spouse, Child or Other, it is required that the correct Relationship to Insured is selected and that the fields on the left side of this window are completed.
