Physician’s Name – Box 33

This box should contain the full formal name of the provider, whether this is a doctor or other therapist. Any place in the ChiroSuiteEHR system where the provider’s formal name is used, it will be pulled from this entry. This includes but is not limited to practice management reports, letters, forms, SOAP notes, and narrative reports. For box 33 on the CMS-1500, there is an option in the Insurance catalog to select either this entry or the Clinic Billing Name.
