The information on this window is specifically for New York State Workers’ Compensation claims. Entries should be made on this page only when the practice is treating a New York Workers’ Compensation patient.
The State of New York mandates the completion of specific forms for Workers’ Compensation claims. These forms are the C4.0 for an Initial Claim and the C4.2 for follow up claims. The ChiroOffice section of ChiroSuiteEHR has the ability to produce both forms. The first requirement to produce these forms is to complete all the fields on the New York State C4 Tab (figure 11).
In order to generate the forms, in the patient’s Insurance Policies Billing Tab window, verify that New York C4 in the Form Type drop down box has been selected.
