If the provider’s name has been entered in the Initials box in the Provider Catalog, then the provider’s Initials will appear in this drop down box. Many years ago only the actual initials were required. At this time, Medicare and State Boards mandate that the provider’s full name must be displayed. The name that appears in the Initials box will be generated at the end of the SOAP note as an electronic signature of the SOAP note. In order to comply with Medicare’s requirements, the name in this box should include the provider’s credentials and in Settings, the selection must include a check in the box to Use Extended Format.
