D.O.B.

The D.O.B. box is for the Subscriber Policyholder’s date of birth. It frequently occurs that this information is not available when it is needed. The system requires an entry in this field whenever the relationship is Spouse, Child or Other. If Subscriber Policyholder’s date of birth is NOT available and it is desired to leave the field blank on the insurance CMS-1500 claim form, click into the DOB field and then push the space bar on your keyboard. This will enter 00-00-0000 letting the program know that the box has not been ignored. When the CMS-1500 prints, it will leave the insured’s date of birth blank.
