This is the date that the patient first Consulted in the practice for the current condition. On the current paper CMS-1500 form there is no field for this date to be entered. However, when it is submitted to electronic clearing houses, the Consulted date may be transmitted. It is important to verify that Consulted date is on or after the Onset date.
