This box identifies what the patient’s current condition is related to. It will also determine which insurance program is covering the patient. That is, if none of these items is checked, then it will be the responsibility of the regular health insurance. If Employment is checked, then it will be deemed to be a Workers’ Compensation claim. If Auto Accident is checked, then it should be going to car insurance. If Other Accident is checked, then whichever insurance carrier it is sent to, it will trigger additional paperwork to substantiate that the carrier it was sent to is actually responsible for the coverage.
