This is a simple text box for the entry of special information required by the insurance company. In the past it was used exclusively by Medicare and Medicaid. Recently other insurance carriers have started requiring information in this box. Just type in whatever the insurance carrier requires. However, if it is for Medicare, the entry will be made based upon selections made in the Medicare settings in the Insurance Carrier Catalog.
