Adding a Referring Physician

In order to Add the Referring Physician, click on the Add button at the bottom of the box, find the appropriate Referring Physician in the catalog, and double click on that doctor’s name.

This number is provided by Medicaid, and is used in box 22 on the CMS-1500 form. Just type in the number when it is given to the practice.
