Addendums

Muscle Test reports from previous dates can NOT be edited. If there is a reason to make another entry in that report, it is required that it be done as an Addendum. This enforces the strict control over all Muscle Test reports and provides bullet proof authenticity. It eliminates the ability of anyone to falsely accuse anyone in the practice of “doctoring” the records.

 

After the Muscle Test has been saved, it can be changed, edited, or modified ONLY by using the Addendum tool (figure 56). Click on the Addendum button at the top center to open the Addendum window. Note that default information is entered automatically, showing the Date of the Addendum, the Initial Report Date, and which member of the office staff is making this entry. Required data that must be entered includes the name of the doctor that is making this Addendum, the Addendum itself (the information being added to the report), and the Reason the Addendum is being made. This format is required at both the State and Federal levels.

 

After completing the Addendum entry, be sure to click the Save icon at the bottom of the Addendum window.
